Somerset Medical Center works collaboratively with Somerset Hills Hospice, the hospice program of the Visiting Nurse Association of Somerset Hills, to provide hospice services for patients.

The hospice program offers terminally ill patients and their families special help in coping with the physical, emotional and spiritual challenges. The program enables patients to remain in the comfort of their own home during the final phase of their illness to be cared for by family and friends with the support of medical professionals and trained volunteers.

The hospice team, which holds weekly interdisciplinary meetings, includes:

• Physicians
• Nurses
• Social workers
• Pastoral care counselors
• Certified home health aides
• Hospice-trained volunteers
