Outpatient Diagnostic Testing and
Procedures.
For all testing and procedures, please ensure
that all scripts and/or requisition forms include and ICD-9 diagnosis*
and
CPT procedure code*.
*Absence of these codes may result
in additional waiting time or requirement of payment from your
patients.
